Appointment Cancellation Policy
Cancellations must be received 24 hours in advance of the appointment time or clients may be required to pay a no-show fee of a minimum of $30 plus HST. The Wellness Clinic may consider exceptions for emergencies and bad weather if the clinic is notified of the no-show before the agreed-upon appointment time.
All Sales Final
Refunds are not provided on services or products purchased at the Wellness Clinic. All sales are final.
Privacy PolicyThe appropriate collection, use and disclosure of patients’ personal health information are fundamental to the day-to-day operations of the Wellness Clinic and to patient care. Wellness Clinic staff collect only that information which may be necessary for purposes related to the provision of your medical care, therapy or skin care treatments.
Access to personal information will be authorized only for the therapists, physicians and employees associated with the clinic on a need-to-know basis, and other agents who require access in the performance of their duties, and to those otherwise authorized by law.
When we provide information to health care providers acting on your behalf we only give them the information necessary to perform the services for which they are engaged. Clients of the Wellness Clinic may obtain a copy of the information contained in their file on request with payment of a nominal fee to cover the cost of gathering and copying the required material.
